Creating Healthy Communities—2016 Success Stories
The Ohio Department of Health implemented the Creating Healthy Communities Program—funded predominantly by the Preventive Health and Health Services Block Grant—to improve the health of Ohioans and prevent chronic disease statewide by increasing opportunities for physical activity, improving access to and affordability of healthy food, and ensuring tobacco-free living. In 2016, the CHC program impacted more than 700,000 Ohioans in 23 counties (both rural and urban) and leveraged more than $4 million to support these sustainable changes.
