Federated States of Micronesia Uses Block Grant Funding to Provide Better Care for Diabetes Patients
One in three residents in the Federated States of Micronesia (FSM) has diabetes. Limited resources and patient access to care added to FSM's public health challenges. With funding from the Preventive Health and Health Services Block Grant, FSM introduced a Chronic Care Model to train its public health workforce to manage chronic diseases, share information and improve patient care across health organizations, track the number and types of services provided, and reach more patients with timely and coordinated care. More than 130 staff members are now trained and ready to implement the model and provide essential services to diabetes patients to prevent potentially life-threatening complications.
