On or about August 7, 2024, SW Family Medicine became aware of a security incident on its IT Network. SW Family Medicine 's investigation discovered that the following types of sensitive personal information and protected health information may have been exposed in the data breach: Full name, Social Security number, driver 's license number, date of birth, address, insurance information and medical information. On November 7, 2024, SW Family Medicine issued a public disclosure about the data breach and started sending notice letters to individuals impacted.
