Seniors in Azusa, California will have a home in a new 96-bed care facility, which will offer meals, mental health, medical care, and case management services to address their urgent needs. SACRAMENTO, Calif., Oct. 1, 2025 /PRNewswire/ -- Health Net, one of California's most experienced Medi-Cal managed care health plans, and the Centene Foundation, the philanthropic arm of Centene Corporation (NYSE: CNC), today announced a partnership investment of $7 million to support The Wesley Azusa Housing & Health Program. The program includes a 96-bed facility that will provide comprehensive wrap-around services for unhoused individuals over the age of 55 who experience ongoing medical care needs. "Health begins with dignity," said Dorothy Seleski, Medi-Cal president at Health Net. "True wellness means more than access to medical care. It means having a safe place to sleep, nourishing food to eat and the comfort of knowing you are seen and valued. When we come together to uplift those who need it most, we create communities built on compassion, strength and hope." The 27,300 square-foot residential care facility, 204 W. Paramount Street, will serve as a permanent home while also offering a comprehensive range of supportive services. These services will include:

"Through this partnership, we're continuing in our commitment to improve the health of under-served community members through care coordination, health education, and other medical services." Set to open in 2026, the Housing & Health Program will leverage collaboration between the Los Angeles County Department of Health Services, managed care providers, and other community partners to help identify clients experiencing homelessness and connect these individuals to the program for immediate intake. "The investment from Health Net and the Centene Foundation demonstrates the kind of transformative care that is possible when we come together for our most vulnerable neighbors," said Los Angeles County Chair Pro Tem and First District Supervisor, Hilda L. Solis. "Everyone, including our unhoused and older adults, deserve access to safe housing along with essential medical care and wraparound services. We've already seen great success in partnerships with Wesley Health Centers to improve the health of residents, and I know this program will help address a critical gap for our unhoused older adults, so they can move towards a pathway of stability, and hope." From 2017 to 2024, Health Net committed more than $158 million in funding to community-based organizations, including dedicated funding for housing-related initiatives.
